A basilica founded around 417 and rebuilt many times, where on 26 December 1476 Duke Galeazzo Maria Sforza was stabbed to death by aristocratic conspirators; a plaque set into the floor marks the spot. Caravaggio was baptised here; inside, look for Fede Galizia's early 17th-century Noli me tangere and Procaccini's Transfiguration.

Good to know
- Since 2015 it is the parish for Milan's migrants, with Masses in Filipino and Spanish.
- It shares the square with San Bernardino alle Ossa and its bone chapel, so see both together.
- The lone column in front of the bell tower is the last piece of the old narthex.
